New Players Dues + Waiver

ACKNOWLEDGMENT AND ASSUMPTION OF RISKS

I, the undersigned, wish to participate in rugby practices, games, fundraising events, and related travel organized by the Columbus Coyotes. I acknowledge that participating in rugby and related activities involves physical exertion, a contact sport, inherent risks, and dangerous field conditions that may result in physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss, and/or death. I understand that these risks may arise from my own actions or inactions, the actions or inactions of others participating in the event, the conditions in which the event takes place, or the negligence of the released parties. I also acknowledge that injuries or outcomes may arise from rugby participation, including severe injuries and death, as well as from travel to and from event locations.

I hereby assert that my participation is voluntary and that I knowingly assume all such risks, including but not limited to those related to field conditions, practices, games, fundraising events, and travel.

RELEASE AND WAIVER OF LIABILITY

In consideration of being permitted to participate in rugby practices, games, fundraising events, and related travel ("activities"), I hereby release, waive, discharge, and covenant not to sue the Columbus Coyotes, its directors, officers, employees, volunteers, agents, and successors from any and all liability, claims, demands, losses, or damages on my account resulting in any way from my participation in the activities, including any negligence of the Columbus Coyotes, its directors, officers, employees, volunteers, agents, or anyone allegedly acting on its behalf, including negligent rescue operations. This waiver and release shall cover all activities including but not limited to injury, disability, death, or loss or damage to person or property, whether arising from the condition of the field, participation in rugby activities, fundraising events, or travel to and from such activities.
